An innovative pilot project to identify and help the people most at risk of heart disease, which included patients across all GP practices in Peterborough, has been shortlisted for a national award.
NHS Peterborough experiences higher death rates from heart disease than national rates; based on this need, it became an early adopter of the Department of Health’s National Vascular (heart) Checks Programme. This is a programme of heart risk assessment and management through GP practices for those aged between 40-74.
The project has been shortlisted for the PharmaTimes Joint working project 2010 which recognise good examples of Joint Working Projects. The nomination is a culmination of work by NHS Peterborough, pharmaceutical company Takeda and the Anglia Stroke and Heart Network.
Chas Ryan, improvement and development manager at NHS Peterborough, said, “We’re delighted to be shortlisted for this award which shows the importance of joint working and how it can be vital to the progress of a project.
"Through a proactive approach to our project, a significantly and previously unassessed group of patients have received health advice, reassurance, and where appropriate managed heart disease at an earlier stage than would occur otherwise.
“Part of our five-year strategic plan is to reduce deaths and numbers of people suffering from heart disease, and we hope that the work carried out in this project will contribute towards this.”
The award ceremony takes place on Thursday 1 July 2010 at the InterContinental Hotel, Park Lane, London.
